Identifying the optimal patient journey by leveraging the experience of all care givers across different patient care settings 

Abstract
Every day, millions of people are admitted to hospitals, embarking on patient journeys on the road from sickness to health. These patient journeys are at once unique and complex, with many potential paths along the way. 

Many things impact the ability of the patient to heal, with the most important being the patient’s care team. Including each team members’ individual perspective, experience and knowledge.

Approach
Connected all care givers in a collaborative methodology which redesigns clinical workflows tailored to each individual patient journey. Prototyped and iterated ideas over the course of 2 years in close collaboration with countless care teams in hospitals.

Outcome
Created a digital solution in order to assess, understand and map the current state of caregiver workflows. Explore, discuss and simulate new ways of working. Compare patient-specific workflows to identify the best clinical pathway and apply the best clinical pathway and measure success.

Shortlisted for IxDA Interaction Award 2018.
